Paperwork for Fall 2025 medical clearance process will be available on July 1, 2025! 

Each year prior to participation in Lesley Athletics all student-athletes are required to completed medical and insurance forms, in addition to their pre-participation screen with the Athletic Trainer. These forms can be completed online using our medical record system SportsWare. If a student-athlete has not been cleared by a Lesley University Athletic Trainer/Team Physician they will not be allowed to participate in any athletic activity. Please note that in addition to the annual athletic medical clearance, all individuals will be required to follow all conduct expectations and health protocols of the university. Required Forms in SportsWare Checklist

All forms and paperwork must be completed and uploaded to your own SportsWare account by August 1, 2025.  If you have seen a health care provider outside of Lesley within the last 12 months you will be required to upload additional paperwork in SportsWare in the "FORMS" section before your Pre-Participation meeting with the Head Athletic Trainer.

•  2025-2026 Physical All NEW student-athletes must have a physical completed by a physician within 6 months of September 15, 2025, per the new NCAA guidleines.  All RETURNING Lesley student-athletes must have a physical completed by a physician within 12 months of August 1, 2025. This physical is not able to be completed through Lesley University. Documentation must state that the individual is cleared for participation in competitive interscholastic athletics. A letter from your doctor stating that you had a physical and are cleared for athletics does not meet the requirement of a physical exam.  Please upload a completed copy of this exam to SportsWare, LLC in the "forms" section.  

• Sickle Cell Testing Requirement - Per updates to the Division III NCAA requirements, all NEW, Incoming student-athletes (freshman and transfers) are required to provide proof of sickle cell trait test.  Proof of Sickle Cell Trait status MUST be uploaded to student's SportsWare account in the attachments section. This requirements can be met in the following ways: 

1) Newborn Screening Test Results: Individuals can look up how to find their sickle cell trait status from birth and who to contact by state here.  Most states required newborn SCT testing beginning in 1990.

2) Get tested: Provide a copy of a recent Sickle Cell Solubility Test indicating their Sickle Cell Trait status. This requires a blood test through a physician's office or may be completed through a third-party lab for a small fee such as this one here.  The results of the test must be complete and uploaded prior to having a Lesley Medical Clearance appointment in August 2025..

• Medical Clearance Paperwork Packet In SportsWare - This year the majority of the paper work has been consolidated to a packet that is available on SportsWare.  It is your responsibility to read through this packet, fill out the necessary information and sign in the spaces provided.  If this is not completed, or there is missing information, the packet will be rejected on SportsWare and you will need to make the necessary changes to resubmit.

• PHQ-9 Survey in Sportsware - This is a mental health questionairre that is required for all student athletes.  Please make sure you take a few moments to answer these questions thoughtfully and honestly.

• Baseline Concussion Testing SWAYAll Freshman, Juniors and Transfer student athletes must have a baseline test on file with the SWAY Medical app.  This testing will be completed under the direction of the athletic trainer with your team once you have moved on to campus. Students will be eligible for the testing after they have completed all paperwork, made a pre-participation appointment with the Athletic Trainer.  All athletes are required to download the SWAY app on their phone and keep it accessible on their phone during the school year.

• ADD/ADHD Medical Exemption (If Applicable) As a member of the NCAA, we are governed by their rules and regulations regarding drug testing. Beginning August 2009, stricter policies were put in place regarding documentation for medical exemption of banned stimulant medications used to treat ADD/ADHD. This form and all listed documentation must be completed by the prescribing physician, and uploaded to patient's SportsWare account prior to participation in Lesley athletics if applicable. This form must be completed and signed by your MD if you are taking any medication for ADHD. Please upload the completed and signed form to SportsWare if you are taking medication for ADHD.

All Lesley student athletes are required to take an online training course for concussion education.  This online training must be completed and your certificate uploaded to SportsWare by August 1, 2025.  Lesley has partnered with Concussion Awareness Training Tool for coaches and athletes to complete training. SWAY Medical is the app used to conduct baseline concussion testing.  This testing will be performed after you have completed your Medical Clearance appointment. All freshman, juniors and transfer students will be required to take a SWAY test with the Head Athletic Trainer. DOWNLOAD the app to your phone but DO NOT put any information into the app.  Please wait until your group meets for SWAY testing scheduled by the athletic trainer.
